We welcome new patients to join the Practice who have recently moved into the Practice area which consists of BURNLEY, CLIVIGER and WORSTHORNE.

On the right you will see instructions on how to register as a patient.

Named GP for ALL Patients
New service for patients

As from April 2015 all practices are required to provide all their patients with a named GP who will have overall responsibility for their care and support.

What does this mean for our patients?
You will be allocated a named GP who will have overall responsibility for the care and support that our surgery provides to you. Please contact the practice if you would like to know your allocated GP. If you have a preferred GP we will make all reasonable efforts to accommodate your request.

Your GP will work with you and other relevant health and care professionals, who are involved in your care, to ensure that your care package meets your individual needs.

This does not prevent you from seeing any GP in the practice as you currently do.

In addition, for patients aged 75 and over the named accountable GP will also:

- Work with relevant associated health and social care professionals to deliver a
multi-disciplinary care package that meets the needs of the patient
- We are a paperlight practice, which means all your medical history is recorded on the computer.
- We can assure patients of complete confidentiality. Your rights are also protected by the Data Protection Act, which the practice is registered to.

If you change circumstances, i.e. address, name or telephone number, it is important that you inform reception as soon as possible so that your details can be amended. Alternatively fill in the online form here.

Using EMIS Access
We now offer patients the use of a secure web site on the Internet to:

- Request repeat prescriptions online
- Book, view & Cancel Appointments online (24 hours a day / 7 days a week)
- View your medical record – we use GP2GP therefore new patient records can be transferred electronically

You must be a fully registered patient, over 16 years old, and need a letter from us containing personalised access codes . Once you have the access codes and instructions you can connect to the web site at any time and register.

You can visit this page by clicking on the link at the bottom of the homepage. The first time you use the site you will be asked to create your account. Here you will need to enter the four registration codes given to you by the practice.

If one of our doctors has arranged a repeat prescription for you, and the review date has not expired, then you can use EMIS Access to request a repeat. Prescriptions can be collected two full working days after request. Once you have logged onto EMIS Access you can view any medication items by clicking List of your repeat medication in the Repeat Prescriptions section of the screen. Tick the items you require and then click Make Request. There is no need to add a message relating to your request. You will then see a confirmation screen, providing you are happy with the selection you made, click Confirm.

You will be able to access the appointments booking facility. The on-line appointment booking facility currently offers appointments with doctors only. You can book / check or cancel your appointment 24 hours a day. You will have a selection of doctors’ appointments to choose from, up to three weeks in advance.

Some types of appointment will still need to be made via our receptionists, for example nursing appointments, baby checks, minor operations, coil fittings, or complex / multiple problems that may require a double appointment (the standard length of a doctor’s appointment is 10 minutes).
If you have a problem which is medically urgent, then we advise that you contact us by phone in the usual way.

We regard security and confidentiality as extremely important. Communications to the practice are encrypted, and security is at the same high level as for internet banking. It would only be possible to access your records by using your chosen PIN number / password. Needless to say, do not disclose your pin number to anyone. Each patient wishing to take advantage of this facility needs to sign individually a consent form, stating that they agree to their record being available to them on-line.

How to Register

    Download and fill in our Practice Registration form.

  2. Step 2

    Download and fill in our Patient Questionnaire.

  3. Step 3

    Come into the Practice with the above forms filmed in and a form of photo id (such as a driving license or passport) along with confirmation of your address (such as a utility bill).
